More than four years later, the public health emergency continues as the province responds to a second public health emergency: the COVID-19 pandemic. <br><br>Prior to COVID-19, overdose deaths in BC were <a rel=\"noreferrer noopener\" href=\"https:\/\/www.bccdc.ca\/about\/news-stories\/stories\/2020\/overdose-fatalities-unacceptably-high\" target=\"_blank\">declining<\/a>, even as rates of non-fatal overdose remained high. As street drugs became increasingly toxic, interventions like Take Home Naloxone (THN) and Overdose Prevention and Supervised Consumption Sites (OPS\/SCS) prevented nearly <a rel=\"noreferrer noopener\" href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/add.14664\" target=\"_blank\">6,000 deaths<\/a>. <br><br>While public health used legal orders to make interventions like THN, OPS, and <a rel=\"noreferrer noopener\" href=\"https:\/\/www.bccsu.ca\/wp-content\/uploads\/2020\/04\/Risk-Mitigation-in-the-Context-of-Dual-Public-Health-Emergencies-v1.5.pdf\" target=\"_blank\">pharmaceutical alternatives to the toxic drug supply<\/a> available, much of the immediate frontline response to the overdose emergency fell to communities of people with lived and living experience (PWLLE) of substance use and peer-run organizations which often operate services without sufficient and sustainable funding or organizational support. <br><br>\u201cPWLLE are first responders. We\u2019re the first people on the scene, who are there while it\u2019s happening, who keep people alive until the government responders get there,\u201d says Marnie Scow, who works with Professionals for the Ethical Engagement of Peers (PEEP) at the BCCDC. <br><br>As PWLLE respond to the overdose emergency, they are also directly affected by it.&nbsp;Many PWLLE face the reality of overdose risk themselves, and are exasperated by the overwhelming and continuous loss of family, friends, and loved ones. <br><br>Since BC declared COVID-19 a public health emergency, <a rel=\"noreferrer noopener\" href=\"https:\/\/www2.gov.bc.ca\/assets\/gov\/birth-adoption-death-marriage-and-divorce\/deaths\/coroners-service\/statistical\/illicit-drug.pdf\" target=\"_blank\">overdose deaths have reached an all-time high<\/a> in the province. <a rel=\"noreferrer noopener\" href=\"https:\/\/www.bccdc.ca\/health-professionals\/data-reports\/overdose-response-indicators#OPS\" target=\"_blank\">Visits to OPS\/SCS have dropped<\/a> since March, as sites closed temporarily or operated at reduced capacity to limit the spread of COVID-19. <br><br>At times, responses to the dual public health emergencies have been at odds. Although overdose deaths have consistently outpaced COVID-19 deaths (Figure 1), that\u2019s not what one may expect, given the differences in media attention and scale of response between the two emergencies.<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/pacificpublichealth.ca\/wp-content\/uploads\/2023\/11\/OD-1-scaled.jpg\" alt=\"\" class=\"wp-image-4358\" width=\"512\" height=\"342\"\/><figcaption>Figure 1. BCCDC<\/figcaption><\/figure><\/div>\n\n\n\n<p>\u201cI see the efforts and money that\u2019s been put into COVID-19, and overdose has squashed COVID-19.&nbsp; It took COVID-19 for us to even get safe supply \u2013 it didn&#8217;t happen until lives besides drug users\u2019 lives were at risk.&nbsp; It wasn\u2019t enough that people were dying from overdose at alarming rates,\u201d says Jenny McDougall of PEEP, speaking of Risk Mitigation Prescribing. <br><br>As of September, the <a rel=\"noreferrer noopener\" href=\"https:\/\/news.gov.bc.ca\/releases\/2020MMHA0051-001754\" target=\"_blank\">Ministers of Health and Mental Health and Addictions have approved expanding access to prescribed pharmaceutical alternatives<\/a> to the toxic drug supply: it no longer need reference COVID-19 prevention, and instead emphasizes the need to reduce reliance on an increasingly toxic drug supply. <br><br>While both emergencies have needed intervention from public health agencies, the nature of response that each requires differs greatly. Overdoses require medical intervention within minutes, and often happen in community settings or in housing. <br><br>Governments made use of the opportunity to scale up needed and cost-effective interventions for people who use drugs (PWUD). However, the fact remains that there was not enough political and social support to implement these interventions without COVID-19 prevention as a justification. This lack of political will reinforces stigmatizing messages from society. <br><br>\u201cPeople who use substances have been criminalized to the point where no one cares about their lives,\u201d says Paul Choisil of PEEP. <br><br>\u201cThe difference in scale of response sends us the message that lives of PWLLE are expendable\u2014people who use drugs are not a priority,\u201d Charlene Burmeister of PEEP added. <br><br>Provincial Health Officer <a rel=\"noreferrer noopener\" href=\"https:\/\/www2.gov.bc.ca\/assets\/gov\/health\/about-bc-s-health-care-system\/office-of-the-provincial-health-officer\/reports-publications\/special-reports\/stopping-the-harm-report.pdf\" target=\"_blank\">Dr. Bonnie Henry\u2019s position on the topic<\/a> is clear: substance use in our society should be treated as health issue rather than a moral or criminal one. <a rel=\"noreferrer noopener\" href=\"https:\/\/www.timescolonist.com\/news\/local\/calls-for-safer-supply-of-drugs-as-b-c-records-175-overdose-deaths-in-july-1.24192109\" target=\"_blank\">Chief Coroner Lisa Lapointe<\/a> and the <a href=\"https:\/\/www.cacp.ca\/news\/canadas-police-leaders-recommend-adopting-a-public-health-led-diversionary-approach-to-illicit-subst.html#:~:text=VANCOUVER%2C%20BRITISH%20COLUMBIA%20%E2%80%93%20The%20Canadian,endorsing%20the%20decriminalization%20of%20personal\" target=\"_blank\" rel=\"noreferrer noopener\">Canadian Association of Police Chiefs<\/a> echo Dr. Henry\u2019s sentiments. <br><br>PWLLE of substance use are moving society towards recognizing that caring for PWUD is an end in and of itself\u2014it\u2019s not just COVID-19 prevention. The <a href=\"https:\/\/towardtheheart.com\/peer-engagement\" target=\"_blank\" rel=\"noreferrer noopener\">Peer2Peer initiative at the BCCDC<\/a> identifies and works to implement evidence-based interventions to support PWLLE responding to the overdose emergency. <br><br>As PWLLE put overdose prevention into action, and work to bridge the gap between communities and Health Authorities, they\u2019re preventing overdose deaths and disproving stigmatizing beliefs about PWUD. <br><br>As Jess Lamb of PEEP stated, \u201cWe\u2019re gonna do what we need to do to keep people alive.\u201d<\/p>\n\n\n\n<hr class=\"wp-block-separator\"\/>\n\n\n\n<p>One of the initiatives led by the BCCDC, in partnership with the First Nations Health Authority, is <strong>Compassion, Inclusion, and Engagement (CIE)<\/strong>. The BCCDC Foundation for Public Health has partnered to raise awareness and funds for CIE. <a rel=\"noreferrer noopener\" href=\"https:\/\/bccdcfoundation.org\/the-overdose-crisis-covid-19-support-peers-through-two-emergencies\/\" target=\"_blank\">Learn more<\/a> about CIE and how you can support peers doing life-saving
